For most Americans, Medicare is the first encounter with a federal benefits program that requires active decisions under strict deadlines. Unlike Social Security, which can begin automatically, Medicare enrollment requires you to understand eligibility rules, coverage options, and timelines โ€” often while navigating other major life changes like retirement or leaving employer coverage.

The rules are not simple. Turning 65 does not automatically mean you should enroll. If you or a spouse has qualifying employer coverage, enrolling in certain Medicare parts may not be necessary โ€” and enrolling in the wrong part at the wrong time can create lifetime premium penalties. For individuals who receive Social Security Disability Insurance (SSDI), Medicare eligibility begins after a 24-month waiting period, creating an entirely separate set of decisions.

Common Medicare Questions

When should I enroll in Medicare?
Your Initial Enrollment Period (IEP) opens three months before the month you turn 65 and closes three months after. If you have qualifying employer-sponsored coverage from a current employer, you may be able to delay Parts B and D without penalty. The decision depends on employer size and whether coverage is primary or secondary to Medicare โ€” it's worth discussing with a specialist before assuming you should wait.
What happens if I keep my employer coverage past 65?
If your employer has 20 or more employees, your group plan is generally primary to Medicare, and you may delay Part B without penalty. When employment ends or coverage ends, you receive a Special Enrollment Period of up to 8 months to enroll in Part B. However, COBRA is not considered "employer coverage" for this purpose โ€” if you elect COBRA, the Special Enrollment Period clock starts at the end of active employment, not when COBRA ends.
How do Medicare and Social Security work together?
They are separate programs with separate eligibility rules. You can begin collecting Social Security retirement benefits as early as 62, but Medicare eligibility generally begins at 65. If you are already collecting Social Security when you turn 65, you will be automatically enrolled in Medicare Parts A and B. If you are not collecting Social Security, you will need to actively enroll. For SSDI recipients, Medicare coverage begins 24 months after disability benefits begin โ€” regardless of age.
What happens if I miss my Medicare enrollment period?
Missing your enrollment period without qualifying coverage results in permanent premium penalties. Part B carries a 10% penalty for each full 12-month period you were eligible but not enrolled. Part D carries a 1% per month penalty based on the national base beneficiary premium. These penalties continue for as long as you have Medicare coverage โ€” making timely enrollment one of the most financially significant retirement decisions you can make.
